BOARD OF EDUCATION CREATES CENTERS
In 1990, the Pendleton County
Board of Education voted to create school-site centers to address
student health issues. These issues include student illness, school
dropouts, sexually transmitted diseases, drug and alcohol abuse,
weight control, peer acceptance, personal conflicts and teen pregnancies.
Since March 1991, the Centers
have been staffed by a nurse practitioner, physician assistant and
a pediatrician two to three days per week. Pendleton County school-site
centers, the first in West Virginia, served as a model for other
counties.
Parents, teachers,
community health care providers and students are working together
to benefit the health, education and future of our students. Through
the services offered at the Health Centers and with the support
of the community and parents, we can promote more responsible health-related
decisions by students. We will continue to work to improve our children's
self-esteem, ability to help themselves and access resources for
self-help.

Who is eligible to use the Health Center?
Any student may use the health education services of the Center.
Only those students who have returned a consent form may be treated
at the center.
Will parents have a
voice in the Health Center's operation?
Yes. A local school Health Advisory Council is established to ensure
student and parent participation in the formulation of the Health
Center's programs and policies.
Is student information confidential?
Yes. Student's medical records are not included in the student's
school file.
Will students or parents be charged for Health Center
services?
Only services that would normally be billed at a clinic will be
billed to the family. If possible, parents will be contacted before
billable services are delivered. The Center will bill insurance
companies, if desired by the family. If a student has no insurance,
the student will be billed according to his or her ability to pay.

PARENT CONSENT FORM
Parents or guardians will be required to
sign a consent form which authorizes a student to use the Health
Center services. This is available to be downloaded in PDF: Parents'

Students need to have returned a completed Parents' Consent Form
to be eligible to use the health center services.
The Health Center staff will encourage students to discuss their
health care needs with their parents.
